Rhinoplasty

The fast development of more atraumatic and refined operative techniques in the last decade, and the importance of good long-term results and reduced number of failures in nasal surgery, emphasize the need for an up-to-date, step-by-step operative guide. Although there are many good books on the subject of rhinoplasty, there is a lack of practical guides covering surgical anatomy, pre- and postoperative management, and basic techniques for septal-, tip- and osseocartilaginous vault surgery, especially for the less experienced rhinosurgeon. The first edition of Rhinoplasty was published in 1993 and the second edition five years later. The reviews in leading journals were overwhelmingly positive. Any small points of constructive criticism acted as a stimulus to improve the book, and the third edition, nearly twice as big as the first one, shows that fact. It expands and enlarges the canons of nasal surgery elegantly demonstrated in the former editions. In comparison with the second edition, six new chapters have been developed, including expanded and detailed discussions of septoplasty, ethnic rhinoplasty, refinements of the open approach, alar batten grafting and an enlarged section on nasal reconstructive techniques.

Surgical Management of Nasal Obstruction: Facial Plastic Surgery Perspective

Within the specialty field of otorhinolaryngology and head and neck surgery, subspecialties have developed that focus on specific types of problems. We are all familiar
with the various subspecialty fields and recognize that there are areas of overlap. The surgical treatment of nasal obstruction is a large topic. We have divided this into
two issues, one with a primarily rhinologic flavor, and this issue with a more facial plastic perspective. Of course, there is overlap; furthermore, we expect that all of
the subjects discussed will be familiar to all otorhinolaryngologists. Nevertheless, otorhinolaryngologists and facial plastic and reconstructive surgeons
who have focused their energies on functional and cosmetic septorhinoplasty do offer a unique perspective and experience of the surgical management of nasal obstruction,
and this perspective is highlighted in this issue. Many of the articles focus on complex problems related to the septum—treating the caudal septum, a discussion of complications
of septal surgery including their avoidance and management, and when to consider the open rhinoplasty approach to septal surgery. Surgical pearls for septoplasty
are also considered. Functional rhinoplasty also naturally receives significant attention in this issue, with consideration of surgical treatment of the saddle nose, treatment of nasal obstruction
in the traumatized nose, treatment of dorsal deviation, and surgical and nonsurgical treatment of the nasal valve. Piriform aperture stenosis, a less common form of nasal
obstruction, is also considered. A number of facial plastic surgical case studies round out this issue.
