Clinical Practice in Small Incision Cataract Surgery

Cataract is currently the main cause of avoidable blindness in the developing world and accounts for about 75% of cases worldwide. Small incision manual techniques for cataract surgery, as described in this book, should be in the armamentarium of every cataract surgeon, whether it be in the United States or less affluent countries. Phacoemulsification has become the primary method of performing extracapsular cataract surgery, yet it is not always appropriate either from a cost perspective or the density of the cataracts involved. Small incision manual extracapsular techniques, the first choice alternative to phacoemulsification, give visual results equivalent to phacoemulsification at lower cost. Surgeons using this technique regardless of what the latest technology to appear is, are performing state-of-the-art surgery for their patients. Fifty-seven chapters cover all aspects of small incision cataract surgery from lens anatomy to various operative techniques, complications, management and recent advances. It is the first alternative choice to phacoemulsification.

Evidence-based Approach in Cataract Surgery

It is absolutely necessary that the practising ophthalmologist bases clinical and surgical decisions on valid data rather than simply on intuition, hearsay or peer-practice, although this is not to denigrate either one's own opinion or those of colleagues. Evidence-based medicine helps to improve clinical or surgical practice by evaluating the quality of clinical evidence and ensuring that only the best evidence from clinical research is used in the management of individual outcomes. Given that cataract patients may experience infection and complications it is best if the surgeon is on top of the current literature for prevention. Moreover, the surgeon should be au fait with the latest phacoemulsification techniques as well as advances in lens material and design. This marvellous publication covers all aspects of cataract surgery succinctly and provides the reader with enough guidance for best patient outcomes.

Retinal Angiography and Optical Coherence Tomography

OCT is a relatively new imaging technique that is becoming increasingly popular among ophthalmologists in both private and academic settings. Imaging has been a slow moving area in ophthalmology for some time, but now OCT is providing another, more detailed source of demonstrable change in the eye, in diagnostic, therapeutic or post-surgical setting. (OCT and ultrasound both measure advancing disease states and post surgical healing. The difference is that OCT shows more subtle changes, particularly post-surgically.)

Ocular Blood Flow and Glaucomatous Optic Neuropathy

It has been suspected since more than a century that disturbed ocular blood flow (OBF) may play a role in the pathogenesis of glaucoma. Reduced OBF has been measured in glaucoma patients by numberless centres with different techniques in different tissues of the eye. And reduced OBF has been proven to be of prognostic value. Nevertheless the implementation of this knowledge into clinical practice has been very slow.

This practical book will help many ophthalmologists to better understand glaucoma. The reading of this book will not only improve patient care but also give the reader intellectual satisfaction.

Nervous Control of the Eye

The eye is our window to the world and the loss of its function has a most profound effect on our ability to interact with the world around us. Despite this there is a tendency to underestimate the range, complexity and subtlety of the processes that sustain its function. This volume presents a view which attempts to draw together various aspects of our understanding of the autonomic neural control of the eye and linked aspects of its pathophysiology. Areas covered include nervous control of the cornea, accommodation, the Edinger-Westphal Nucleus, purinergic signalling together with retinal and corneal transplantation. The objective of Nervous Control of the Eye is to provide researchers with an insight into the relative significance of the different components involved in optic innervation.

Risk Prevention in Ophthalmology

The press, in both professional medical journals and mass media and, is overflowing with conflicting reports and sometimes confusing data about the costs of medical malpractice litigation. Incontrovertible is the fact that for physicians in daily practice, these questions are not merely academic. A lawsuit can wreck havoc on a clinician’s professional and personal life.

In Risk Prevention in Ophthalmology, many of the questions surrounding medical malpractice are answered clearly, concisely and pragmatically for physicians, but physicians. Written with an emphasis on clinical matters, with a minimum of “legalese,” this book shows ophthalmologists how to avoid law suits in the first place. How truly informed are your patients after signing your consent forms? Do your patients have rational, reasonable expectations regarding outcomes? Are your history-taking and record keeping habits leaving you exposed? Are you sufficiently careful with documentation?

Should you find yourself involved in litigation, chapters written by physician attorneys provide insights into both the defendant and plaintiff perspectives. Among the more helpful chapters, “Natural History of a Lawsuit,” walks the reader step-by-step through the procedure. (This chapter is like ground rounds for legal issues.)

Covering the legal issues for all of ophthalmology, Risk Prevention in Ophthalmology provides the busy clinician with the tools necessary to reduce not only the risk of potential lawsuits, but also insights for coping with actual ones.
